Για να μας στείλετε έναν δίσκο ή άλλο αποθηκευτικό μέσο για διάγνωση, ακολουθήστε αυτά τα απλά βήματα:

1. Εκτυπώστε και υπογράψτε το ιδιωτικό συμφωνητικό για την εχεμύθεια.
2. Συσκευάστε το δίσκο σύμφωνα με τις οδηγίες που θα βρείτε εδώ. Μην ξεχάσετε να συμπεριλάβετε το ιδιωτικό συμφωνητικό μέσα στο δέμα. ΠΡΟΣΟΧΗ! Χωρίς το ιδιωτικό συμφωνητικό δεν μπορεί να ξεκινήσει η διαδικασία διάγνωσης.
3. -Προαιρετικά, για δική σας διευκόλυνση- Εκτυπώστε αυτό το packing slip για να στείλετε το δέμα στα εργαστήριά μας στην Αθήνα ή αυτό το packing slip για να το στείλετε στα εργαστήριά μας στη Θεσσαλονίκη και κολλήστε το στο εξωτερικό μέρος του δέματος.
4. -Προαιρετικά- Για να επωφεληθείτε της δωρεάν αποστολής του δίσκου προς την έδρα μας, συμπληρώστε την αίτηση από εδώ. Μόλις το κάνετε, κάποιος εκπρόσωπός μας θα επικοινωνήσει μαζί σας για να επιβεβαιώσει πως το δέμα είναι έτοιμο και τα στοιχεία που έχετε δηλώσει είναι σωστά.
